FIE Data Collection Acknowledgement Form

Note for FIE Licenses Only: To apply for a FIE license, the licensee must sign the FIE Data Collection Acknowledgement Form.

You will need to download and sign (by hand) the FIE Data Acknowledgement Form before uploading into the FIE License Application. If the athlete is a minor, both the athlete signature and parent/guardian signature must be included.

The addition of the form is in response to the European Union adopting the General Data Protection Regulation (BDGPR), which applies to all legal entities based in the European Union. As the FIE is based in Switzerland, all FIE licenses now must come with an authorized data release form signed by the licensee. 

This form only needs to be submitted for new licenses (not renewals). 

Form will be attached to application in order to be processed.
